Patient Registration

For the convenience of our patients, you can become pre-registered for Saint John Hospital by obtaining a patient registration form. Individuals can print this off at home and bring it in on their first visit at the Saint John, or fax it to us at 913-680-6188 and we will have them pre-registered in our system for future visits.
