Admission Criteria

Admission to Senior Behavioral Health Unit is indicated for patients ages 55 and older who have a psychiatric diagnosis or symptoms and meet the severity of illness of at least two criteria below:

  • Suicide attempts/ ideation
  • Self-destructive behavior
  • Homicidal ideation
  • Assaultive behavior
  • Destructive to property
  • Medication noncompliance
  • Disorientation or memory impairment causing change in daily living activities
  • Psychiatric symptoms such as: hallucinations, delusions, panic reaction, anxiety, agitation, depression

Documents Needed for the Intake Process

  1. Demographic information– Face sheet
  2. Copy of DPOA/ Guardianship
  3. Physician Order for admission
  4. Progress notes containing information on referral need
  5. Medication list
  6. Insurance cards
  7. Recent UA/LABs if available

Exclusion Criteria

Patients with C-diff, MRSA, VRE and other communicable diseases will be assessed on an individual basis. Patients with primary substance abuse disorder and patients requiring dialysis.
